This module provides access to the official forms used for the Project HOMES
Resettlement Program. Applicants may download, print, and have the forms
notarized in accordance with NHA guidelines.
How to Use These Forms
- Download the appropriate form(s) for your application
- Print the forms on legal or A4-sized paper
- Fill out all required fields completely and accurately
- Have the forms notarized before submission
- Submit the notarized forms along with other requirements

Sworn Application to Purchase Lot/Lots with Housing Unit/Condominium Units (SAPL)
Main application form for the NHA Resettlement Program. Must be notarized.Primary Form

Affidavit of Cohabitation
Required for applicants in common-law relationships to declare cohabitation status.

Affidavit of De Facto Separation
Required for applicants who are separated but not legally annulled.
Important: All forms must be notarized before submission.
Incomplete or unnotarized forms will not be accepted.
